To help us look after you at home we loan you a blood pressure machine, a device to record your ECG called Kardia by AliveCor, and a pulse oximeter to wear on your finger. We ask you to record measurements at home in the morning before 9am and again in the afternoon before 3pm. This information can be added onto either your phone or electronic tablet on a special app "Mydignio" that we provide.
We register you with the system before you are discharged using your telephone number. The information is kept securely on the system and has passed all the NHS security tests. We will give you the loan equipment and show you how it works and talk through your medical plan. We will either give you a prescription or tablets before you go home.
Your condition is reviewed by us at least twice a day and to do this we need you to take measurements via the App. When you open the MyDignio App you will see the tasks and times for the day. Click on the task you want to do and take the measurement . The results are then added automatically if the device has Bluetooth or you can add the result by pressing ’add value’ and click ’submit’. This will mark the task as done and turn it green.
If you start to feel unwell or have symptoms that are getting worse please record an ECG at the time of symptoms and send via the App, this will be reviewed in the next ward round. If you need medical advice sooner, you can either send a message through MyDignio app or call the clinical team. You'll be given clear instructions within your app on what to do and whom to contact if you need urgent advice in-hours or out-of-hours.
Your clinical team is available between the operation hours of 09:00 to 17:00, 7 days a week. During these
hours they will monitor your health twice a day and will contact you either via the messaging system, telephone call or video consultation if there are any changes needed to your treatment. The healthcare team may ask you to attend the hospital if they are concerned about your health.
It is important that you return the loaned equipment as soon as you have been discharged from the virtual ward. This will mean someone else will be ale to have the same care and support you received. The equipment should be returned directly to the clinical team and an appointment will be made with them to do this.
